Merchant customers have full access to the data they collect within the Forma Shopify App, which can be found under the 'Results' tab within the Survey builder.

Consumer opt-out
No personal consumer data is collected other than an anonymous customer ID, however consumers can request deletion of their data and/or future usage or monetization of their data by emailing info@getforma.shop, or calling (949) 632 4813,
